Comparative characteristics of the features of the course of pregnancy and childbirth in women with infertility and obesity in the last 10 years

PURPOSE: comparative characteristics and clinical analysis of the course of pregnancy and childbirth in women with
infertility and alimentary-constitutional obesity over the past 10 years (2009 and 2019).
MATERIALS AND METHODS: analysis of the history of childbirth of obese patients, which were treated and given
delivery in the department of pathology of pregnancy and childbirth of MI “Institute of Pediatrics, Obstetrics and Gynecology
named after acad. E. Lukyanova NAMS of Ukraine» in 2009 and in 2019.
RESULTS: The number of pregnant women with obesity over the last 10 years has increased almost 2.5 times. The
predominant number of pregnant women with obesity on the background of infertility are older reproductive age, combined
somatic and gynecological pathology, hormonal changes in the form of progesterone deficiency and chronic inflammatory
processes. Pregnancy in this group of women is accompanied by a prolonged recurrent threat of miscarriage (in 2009 - 33.3%,
in 2019 - 46.7%), premature birth (27.8% and 40% respectively), placental dysfunction (22.2 % and 26.7%) and preeclampsia
(20% and 26.7%), while every second woman has a combination of several pregnancy complications (52.2% and 62.2%) and
needs repeated inpatient treatment (44.4 % and 55.6%). The frequency of delivery by cesarean section increased by 1.5 times
(32.2% and 48.9%), the most common complication of labor remains premature rupture of amniotic fluid (11.1 and 15.6%).
Thus, the combination of obesity and infertility during pregnancy requires in-depth study and development of differentiated
antenatal observation algorithms.
