Efficacy of combining ultrasound cavitation with intrauterine administration of platelet-rich plasma in patients with thin endometrium

Abstract

Relevance: Reduced endometrial receptivity is considered one of the leading factors of implantation failure in in vitro fertilization cycles. An endometrial thickness of less than 7 mm is commonly used as a criterion for insufficient endometrial proliferation and is associated with lower pregnancy rates. Despite advances in assisted reproductive technologies (ART), the management of thin endometrium remains a relevant clinical problem.

The study aimed to evaluate the effectiveness of combining ultrasound (US) cavitation of the uterine cavity with autologous platelet-rich plasma therapy (PRP) in patients with a thin endometrium.

Materials and methods: The study included 34 reproductive-age women with a thin endometrium (<7 mm). All patients underwent ultrasound-guided cavitation of the uterine cavity, followed by intrauterine administration of autologous platelet-rich plasma (PRP) therapy. Treatment efficacy was assessed by changes in endometrial thickness measured by ultrasound and clinical pregnancy rates.

Results: The mean endometrial thickness increased from 6.08 mm before treatment to 8.52 mm after combined therapy. The implantation rate was 61.8%, while the clinical pregnancy and live birth rates were 58.8% and 32.4%, respectively. Patient age and the presence of intrauterine adhesions were significant predictors of treatment effectiveness.

Conclusion: The use of ultrasound cavitation in combination with PRP therapy significantly increases endometrial thickness and improves the likelihood of pregnancy. These findings suggest that this approach may be a promising option for the management of patients with thin endometrium and recurrent implantation failure.
