Relevance: Antimicrobial resistance among uropathogens in complicated urinary tract infections (UTIs) in pregnant women, especially when combined with urolithiasis, complicates the administration of empirical therapy and jeopardizes the safety of both the mother and the fetus, underscoring the need to update local microbiological data.
This study aimed to assess the nature and prevalence of antibiotic resistance among uropathogens causing complicated urinary tract infections associated with urolithiasis in pregnant women.
Materials and Methods: This retrospective descriptive analysis was conducted from June to September 2023 at a tertiary-level urology center in Kyrgyzstan. The study included 110 medical records of pregnant patients with clinically confirmed complicated pyelonephritis associated with urolithiasis. Urine cultures were performed on CLED agar, as well as on Endo and CHROMagar Orientation media. Microorganisms were identified using the VITEK 2 Compact system and biochemical tests. Antimicrobial susceptibility testing was performed using the Kirby-Bauer disk diffusion method. ESBL production, multidrug resistance, and resistance to selected antimicrobial groups were analyzed using descriptive statistics and comparative tests in SPSS 25.0.
Results: Gram-negative bacteria predominated, especially Escherichia coli (45%), followed by Staphylococcus spp. (10.5%), Klebsiella spp. (10%), Pseudomonas aeruginosa (9%) and Staphylococcus aureus (5.5%). Among Enterobacteriaceae, 58.3% were ESBL producers and 41.7% were multidrug-resistant. The highest susceptibility was observed for fosfomycin, meropenem, and tobramycin (100%), followed by amikacin (85.5%) and levofloxacin (78.8%). Ampicillin, amoxicillin/clavulanate, and trimethoprim showed low activity (<30%).
Conclusion: The findings support local microbiological monitoring, avoidance of highly resistant empirical agents, and culture-guided therapy in pregnant women with complicated UTIs associated with urolithiasis.
