Development and pilot psychometric validation of a comprehensive questionnaire for the assessment of clinical symptoms and quality of life in genital endometriosis

Abstract

Relevance: Genital endometriosis is a common chronic disease characterized by significant pain and reduced quality of life in women of reproductive age. In clinical practice in Kazakhstan, there are no adapted and validated instruments for the comprehensive assessment of disease symptoms and their impact on quality of life, particularly ones that consider biological markers of disease activity.

The study aimed to develop a structured questionnaire for the comprehensive assessment of clinical symptoms and quality of life in patients with genital endometriosis and to conduct a pilot psychometric validation, including a construct validity analysis using immune and biochemical markers.

Materials and Methods: A pilot study was conducted involving 35 patients with genital endometriosis and 20 conditionally healthy women of reproductive age. The questionnaire comprised sections on clinical symptoms, quality of life, and lifestyle factors. Construct validity was evaluated using laboratory immune profile markers (PD-L1, VEGF, IL-6, IL-8, IL-18, TNF, etc.). Internal consistency of the scales was assessed using Cronbach’s alpha, and reproducibility was evaluated using the test–retest method, with the intraclass correlation coefficient (ICC) calculated.

Results: The questionnaire demonstrated good internal consistency (Cronbach’s alpha: 0.79–0.80) and high reproducibility (ICC: 0.87–0.99). Statistically significant moderate correlations were found among clinical symptom scores, quality-of-life indicators, and certain immune markers, particularly IL-8 and leptin, confirming the instrument’s construct validity.

Conclusion: The developed questionnaire is a promising tool for the comprehensive assessment of clinical manifestations and quality of life in patients with genital endometriosis. The results of the pilot study support its reliability and validity; however, further multicenter studies with larger sample sizes are warranted for clinical implementation.
