Assessment of the effectiveness of using urogynecological pessaries for the treatment and prevention of pelvic organ prolapse in postpartum patients: A literature review

Abstract

Relevance: Pelvic organ prolapse (POP) is a widely prevalent condition characterized by the downward displacement of pelvic organs in women—including the uterus, vagina, urinary bladder, rectum, and, in some cases, the intestines—below the level of the hymenal ring, resulting in impaired organ function and a decline in women’s quality of life. Its prevalence varies significantly across different countries, ranging from 3% to 66.8%, which is associated with demographic characteristics, parity, and access to medical care. Analysis of international and regional data indicates that pregnancy and vaginal deliveries, especially multiple pregnancies, instrumental deliveries, and those associated with soft tissue trauma, contribute significantly to the development of POP.

The study aimed to evaluate the effectiveness of using urogynecological pessaries in the postpartum period as a method of early prevention of pelvic organ prolapse.

Materials and Methods: This literature review included studies published from 2016 to 2026. The literature search was conducted using the electronic databases PubMed, Mendeley, and Cochrane Library.

Results: Pessaries have long held an important place in the conservative treatment of pelvic organ prolapse (POP) and stress urinary incontinence. Recent studies confirm their ability to improve quality of life, reduce symptom severity, and enhance pelvic floor stability. Despite extensive clinical experience and the wide variety of pessary designs, new models continue to be developed, and the potential for early postpartum use to prevent pelvic organ prolapse is being explored. Postpartum pessary models have demonstrated safety, good tolerability, and the potential to prevent early pelvic floor weakness. Thus, pessaries represent an effective, minimally invasive, and accessible method for the prevention and treatment of POP, especially in the postpartum period.

Conclusion: Pelvic organ prolapse remains a significant gynecological problem with high and variable prevalence. Urogynecological pessaries have proven effective and safe for both treatment and prevention of POP, including in the postpartum period. However, further research is needed to develop effective preventive strategies that begin in the postpartum period.
